Summary
The Qualified Developmental Disabilities Professional (QDDP) provides direction, development and implementation, direct supervision, and monitoring (observation and evaluation of staff implementing care, service plans & interacting with individuals receiving services) to the service provided. This position has responsibility for approving assessments and individual service plans or treatment plans to ensure that appropriate services are provided to meet the needs of the individuals serviced. They must have documented experience developing, conducting, and approving assessments and individual service plans treatment plan.

Essential Duties and Responsibilities include the following. Other duties may be assigned.
· Supervise the habilitation process of each individual to include: writing the Individual Support Plan (ISP) and Behavior Support Plans (BSP) documents, chairing ISP/BSP meetings, developing training and service objectives, and instructing staff on continuous implementation of ISP’s and BSP’s.
· Maintain frequent and consistent contact with representatives of referring agencies.
· Supervise and develop training for all staff to include: in-servicing and timely submission of new-hire documentation, immediate submission of Payroll change Notices for any changes made in an employee’s schedule/status, scheduling monthly staff meetings, performance appraisals, disciplinary action, and management of daily operations of staffing.
· Execute and provide input for new strategies and programs to attract clients.
· Establish/develop/conduct group home tours as directed by Group Home Director.
· Prepare and evaluate employee job performance, make recommendations regarding personnel action to management, assign staff duties, develop staff schedules, and coordinate staff leave time.
· Monitor and document process of individuals to include: data collection, periodic progress reports, and other documentation as stipulated by state governing bodies or by company management.
· Develop and supervise implementation of ISP/BSP and closely monitor staff for correct implementation and effectiveness. Also responsible for the determination of effectiveness of the program.
· Coordinate activities in the home and community including day training and/or employment and recreational activities.
· Develop and maintain positive relationships with staff, individuals served, families, case managers, and other agencies.
· Manage health and safety of individuals through preventive techniques, provide management of minor illnesses or injuries, and render assistance in medical emergency. Schedule and coordinate medical appointments as needed.
· Maintain and implement ongoing compliance with federal and state regulations/standards including Medicaid Waiver.
· Provide on-site review and oversight of the group home including routine maintenance, vehicle maintenance, and compliance of health, safety, petty cash, and regulatory requirements.
· Manage finances in compliance with the group home budget including overtime, residential and household accounts, grocery shopping, and general management of the group home.
· Attend meetings and in-service training as scheduled and conduct in-service training to direct care staffing as needed.
· Share on-call responsibilities and confer with consultants as necessary.
· This job description is not intended to be all inclusive and the employee will also perform other reasonably related duties as assigned by supervisory and/or administrative personnel.
Qualifications
EDUCATION/BACKGROUND/EXPERIENCE:
· A person with at least one year of documented experience working directly with individuals who have developmental disabilities and who possesses one of the following credentials: (i) a doctor of medicine or osteopathy licensed in Virginia, (ii) a registered nurse licensed in Virginia, (iii) a licensed occupational therapist, or (iv) completion of at least a bachelor's degree in a human services field, including sociology, social work, special education, rehabilitation counseling, or psychology.

· Ability to communicate and interact and to work effectively with a wide range of consumers, family members, and care providers in a diverse community.
· Ability to motivate teams and simultaneously manage several projects.
· Work requires willingness to work a flexible schedule and regional travel.
PHYSICAL/MENTAL DEMANDS AND WORKING CONDITIONS:
· Working conditions are normal for an office and direct care environment.
· Work may require weekend and evening work.
· Work may require infrequent overnight travel
JOB SUMMARY: The DSP is responsible for carrying out each individual's outcomes as described in the Individual Service Plan, as well as assuring that the individual's personal desires are carried out to the greatest extent possible. DSP's provide support to individuals in their day-to-day living in the home and in the community. The DSP assures that the Health and Safety of individuals is maintained and monitor for any changes in health status. DSPs communicate positively and professionally with individuals and their families. DSPs maintain all required documentation of work performed and outcomes completed.
Duties and responsibilities required of the position: Assure that all outcomes are completed for each individual as described in the Individual's ISP. Provide assistance with ADL's as needed, such as but not limited to toileting, bathing, dressing, changing soiled briefs, shaving, brushing teeth, and grooming. Provide adequate nutrition by preparing and serving healthy meals and assisting with feeding as necessary. Maintain a clean and sanitary home environment by regularly sweeping, mopping, vacuuming, cleaning sinks, toilets, countertops and all other areas of the home. Provide assistance, including total assistance if needed, to individuals with domestic duties, such as laundry, making beds, and maintaining clean and orderly bedrooms. Coordinate and maintain personal schedule of individuals by providing transportation to all appointments (ex. medical, work, day support, community groups or churches) Create and maintain professional relationships with individuals, their families, case manager, staff members, and all other service providers. Document all work performed and outcomes completed according to licensing and company standards. Monitor individuals served for any changes in health status, such as but not limited to fatigue, lethargy, changes in appetite, rapid weight gain or loss, and any other significant changes. Notify the Group Home Manager immediately of any changes. Assist Individuals with taking medications according to the "5 rights" of medication management.

Minimum knowledge, skills, and abilities, experience or professional qualifications required for entry level: Have a working knowledge of Developmental Disabilities and strategies for effective communication. Have a working knowledge of Healthcare and how to respond in emergency situations. Ability to communicate calmly and professionally with all people in any situation, including good listening and comprehension skills as well as good written and oral communication skills. Ability to provide assistance to individuals with personal hygiene. Ability to manage and respond to behavioral problems appropriately.
