• Clients must be adults, aged 18 and above
• Clients must have a documented diagnosis of a developmental disability, as defined by applicable local, state, or federal guidelines.
• Documentation may include medical records, psychological assessments, or evaluations conducted by qualified professionals.
• Clients who may benefit from behavioral support services due to challenging       behaviors or social interaction difficulties.
• A history of behavioral challenges should be assessed to tailor support plans effectively.
• Clients must exhibit functional limitations in daily living activities, such as self-care, communication, socialization, or mobility.
• The extent of these limitations should be assessed to determine the level of support needed.
• Clients should face challenges in living independently, which may include difficulties with managing personal finances, preparing meals, or maintaining a safe living environment.
• Clients should have a level of health and medical stability that allows for       participation in the planned activities and services.
• Individuals with complex medical needs may require additional assessments to       determine compatibility with the services offered.
• Guardians or decision-makers of the clients should be in agreement with the       philosophy, services, and goals of the group home.
• A collaborative approach involving families or guardians is important for the success of the individualized care plans.
